Zusammenfassung
Bei einem Großteil der Patienten mit einer Krebserkrankung ist die Lebensqualität deutlich eingeschränkt. Die psychoonkologische Versorgung trägt nachweislich zu einer Verbesserung der Lebensqualität von Patienten in akuter Krebstherapie bei. Die Literatur zeigt, dass bei psychisch hoch belasteten Patienten mit körperlichen Erkrankungen nicht nur die Lebensqualität beeinträchtigt, sondern auch der medizinisch-pflegerische Versorgungsaufwand erhöht ist. Ob sich diese Zusammenhänge auch bei Patienten mit einer Krebserkrankung zeigen, wird aktuell diskutiert. In einer explorativen Studie am Klinikum Herford wurde die Frage untersucht, ob sich Zusammenhänge zwischen der psychischen Belastung und dem Versorgungsaufwand an Kennzahlen im DRG-System nachweisen lassen. Vor allem bei krebserkrankten Patienten mit erhöhter Depressivität zeigen die DRG-Kennzahlen längere Krankenhausverweildauern, vermehrte operative und medizinische Prozeduren und eine höhere Erkrankungsschwere an. Die Studie weist darauf hin, dass in der Psychoonkologie sowohl die Auseinandersetzung mit klinischen als auch mit Fragen der Wirtschaftlichkeit sinnvoll ist.
Abstract
Pilotstudy of inpatient healthcare efforts
For a large part of cancer patients quality of life (QoL) is clearly limited. It could be demonstrated that psychooncological care is conductive to a QoL advancement for patients in acute cancer therapy. As the literature shows somatically ill patients with a high mental burden do not only have a reduced QoL but also higher medical-nursing care requirements. It is an ongoing discussion if these connections are also valuable for cancer patients. An explorative study in the hospital of Herford analyzed the question, if there is a relationship between the psychological burden and the care efforts shown by the characteristic factors of the DRG-system. Especially cancer patients with higher depressiveness values show increased length of hospital stay, more surgical and medical procedures and a higher disease severity according to the DRG characteristic factors. The study indicates that the debate about clinical as well as economical questions is meaningful in psychooncology.
